Name this noise. Throwout bearing?

Have had this terrible noise almost the full life of my jeep. It will sometimes go away for a month or 2. Now it is back every time I drive it. Takes about 5 minutes to start it once everything gets up to temperature. Then it does it at every stoplight. Had the throwout bearing replaced under warranty. It seemed to fix it for a 1000 miles or so and then it was back. What can I do to fix this or lube what is creating it?
